Deleting an out of performance review campaign form

As an HR professional, here are the steps to follow if you notice a form outside the campaign appearing in your "My Actions" widget and you wish to delete it: 

1. Click on the menu
2. Click “Performance” tab to open the window
3. Click "Form Entry" to access the forms
4. Click "Out of campaign entry" to display all formers forms
5. Search for the collaborator's name to find their form
6. Click on the row where their form appears
7. Click "Delete form" to remove it so it no longer appears in your "My Actions" widget
